Registration: All fees
for the main conference and pre/post conference workshops
are
listed on the registration form. Complete the entire form.
Payment in full must accompany
your registration. Please note there are no refunds once you
have registered.

Registration Confirmation: You will receive a confirmation letter and your workshop
schedule approximately 3-4 weeks prior to the conference. If you do not receive it, please contact

Special Needs: If you
have special needs, please indicate this in the appropriate
area on the registration form and a staff member will contact
you.
Certificate of Attendance:
There is a $15 pre-paid fee for Certificate of Attendance.
Lunch: A box lunch is included in the Main Conference fee for Saturday & Sunday. Box lunch includes your choice of turkey, roast beef, vegetarian or vegan wrap.
Parking: Parking is FREE FREE FREE at the Westin Lombard Yorktown Center and there is additional FREE parking at the shopping center across the street.
